TECH REVIEW: THE SKLIP PORTABLE DERMOSCOPE

11/1 - A remarkable new medical-grade digital healthcare device has recently entered the skin care diagnostic market, streamlining the patient triage, consultation and collaboration process.  SKLIP is a portable and affordable alternative to the conventional handheld DERMATOSCOPE, providing skin cancer triage recommendations for medical professionals and health-conscious individuals about a concerning skin lesion.  Founded by practicing dermatologists Asst. (Portland, Oregon), this innovation offers high definition digital dermoscopy imaging from a pocket-sized adaptable solution for any smart phone.  News from Jan, 2022 announces that Sklip, Inc. has received a Breakthrough Designation Device status by the USFDA for the deployment of ARTIFICIAL INTELLIGENCE as part of the next Sklip model.   As part of the company founder’s Dr. Witkowski's mission to “catch the inevitable, early,” a new partnership has developed with Prof. Giovanni Pellacani – President of the World Congress of Dermatology and Chair of the University of Sapienza Dermatology Department in Rome, Italy, who has joined the collaboration team as the lead scientific advisor for development of the hardware device and AI.   This partnership will help empower diagnosticians to better identify potential skin lesions of concern and increase referrals of pigmented skin lesions that require a biopsy.

From an exclusive interview with SKLIP Executive Paul Greer and Founder Dr. Alexander Witkowski, we received a firsthand perspective on the global need for this innovation. Originally launched and widely used in Europe, the SKLIP device is also actively being tested and used in dermatology offices and academic centers across the US.  The current model is the smartphone dermatoscope hardware - limited to scanning, uploading and forwarding of that image.  A major upgrade to the primary model will include software as a medical device (SaMD) AI mole scanning within the publicly available Sklip app and is expected to be available to professionals in 2023 pending FDA Clearance. 

The Sklip App currently allows the user to anonymously communicate dermoscopy images with a dermatologist - someone that actually reads the image for evaluation and then sends an EDUCATIONAL INFORMATION REPORT back. Artificial Intelligence changes that formula.  Dermatology providers can now use this lightweight, easily connected smartphone clip-on dermatoscope.  If there is concern about how to triage the skin lesion, Sklip AI, pending FDA Clearance, would evaluate the digital dermoscopy image based on the modified three-point checklist criteria and categorize the pigmented skin lesion into a suspicious or unremarkable category in under 7 seconds. Around the United States, Sklip can be very useful to nurse practitioners and physician assistants in dermatology and primary care, particularly in rural medical settings says Mr. Greer.

PRE-TREATMENT SKIN PROBE STARTS WITH "EYEBALLING"
For all clinical offices as well as the aesthetics and MediSpa community, having an easy-to-use imaging device (like SKLIP) helps to encourage pre-care or exploratory probing. FAQ: WHAT IS MELANOMA? 
Malignant melanoma, one of the most lethal cancers, is increasing at an alarming rate. It is the leading cause of death in women aged 20-43 years. Importantly, only 1 out of 33,000 moles are malignant and sonography may prevent unnecessary biopsies. The chance of aggression is related to the tumor depth. Tumors less than 1 mm are often cured by biopsy. Disease greater in penetration is readily imaged by 3D Volumetric probes and non-palpable foci of tumor extension or lymph node (glands) involvement are visible in the scanned area. The metastatic potential is measured by the 3D Doppler study and follow up of distant tumor spread may be correlated with serial sonography. Some invasive surgical diagnostic procedures may be avoided by using high resolution imaging since scans detect tumor nests as small as 2 mm in the lymph nodes. This means that a sonogram guided needle biopsy may avoid the necessity of a massive radical operative dissection of otherwise healthy tissues. (See complete review on all Skin Cancers)






Nutrition & the Skin: Choosing the Path of a Certified Nutritionist

The skin performs incredibly complex and vital functions for survival. It’s the first line of defense in protecting the body from infection, allergens, and irritants. Secretions of sebum keep the skin soft, supple, moisturized and hydrated. Touch receptors affect our ability to feel pressure, pain and pleasure. The skin’s surface absorbs whatever is applied to it and an exchange of oxygen occurs through the superficial capillaries of the circulatory system. The skin also synthesizes Vitamin D through the absorption of ultraviolet light.

  
Although we’ve intuitively known this, there is now mounting scientific research that the relationship between a person’s overall health and what their skin reveals is strongly connected. For the skin to function optimally, it needs the right nutrients. There are vitamins, minerals, amino acids, proteins and more that specifically nourish the skin and regulate the release of hormones and enzymes that optimize its functions. And, all of this is tied to how our individualized DNA affects skin aging, moisture levels, UV reactions, and collagen production, for example. 

Rosacea is a baffling skin condition, characterized by skin redness, sensitivity, acne-like papules and pustules, that can progress to a condition called rhinophyma, an overgrowth of tissue on the nose or even affect the eyes. It has a genetic component and is often seen in people of Nordic descent. People who have an infectious process due to the h.pylori bacteria, which can cause stomach ulcers, are often prescribed a medication, defined as a proton pump inhibitor. This is the same medication people take for acid-reflux. Uniquely, people who have rosacea and acid-reflux who are prescribed medication for their acid-reflux notice an improvement in their rosacea symptoms. New recommendations for treating rosacea include having the client take a proton pump inhibitor, intended to reduce acid-reflux.  Once I read about this study, I was hooked! The connection between the gut and the skin is undeniable. 

There is a protein growth hormone called Brain-Derived-Neurotrophic Factor, or BDNF, that functions in the process of learning new information and memory. Some brain disorders, like depression and Alzheimer’s Disease, have been linked to low levels of BDNF. Curcumin is the active ingredient in the spice turmeric. Multiple studies have shown that supplemental curcumin increases levels of BDNF which could lead to potential in treating these disorders.[2]

I’m a late bloomer and an early adapter. That’s why I’ve decided to add to my skillset and become a certified nutritionist. I was in my 40’s when I decided to chuck the nursing career and attend esthetics school. Despite an initial substantial income loss, following my heart and my gut is a decision I have never regretted. I was in my 50’s when I decided to jump into the deep end of the entrepreneur pool. My successful medspa transitioned into owning an esthetics school and empowering others to success in their career passions. And, now in my mid-60’s, I’ve enrolled in an online college program that will guide me to a stronger understanding of nutrition, hormones and DNA. And I plan to build that knowledge into a stronger esthetics program because I believe that an understanding of the connectedness of our skin to every other function in our body is the future of professional skincare. 

So, I am confident that getting certified in nutrition is going to create a stronger esthetics program. Skincare professionals need to have a knowledge of nutrition and I want to create a protocol for a skincare consultation that includes probing deeper into our clients’ genetics and diets and creating individualized treatment plans based on that data.  I don’t believe esthetics education is about beauty. Its about health. Nutrition plays a significant role in homeostasis in our bodies. When our bodies are out of balance, it is evident in the skin. The truth is that we are what we eat!

HealthTech Review: NEUROVINE Portable EEG for Cognitive Pacing



OVERVIEW:
Right out of the gate, I boldly attribute the success of any product testing to the complete know-how and calming demeanor of  a professional training team.  As a professional educator by trade, I recognize the Neurovine instructors' deep insight on the many types of brain issues and how to communicate with head injury sufferers.  Kudos goes to the impressive temperament of the virtual training staff for their calm approach to technically explaining the process-- with compassion about the anxieties of learning something unusual and stressful.   Learning this device requires true bedside manner for so many of us.

DAY ONE: SETUP
After finally receiving the Neurovine kit (complete with the power cord and saline dispenser), I could not wait to charge the device for the 24‐hour recommended charge time so I could pair it to my cell phone and get started right away.  I was somewhat impressed at the quality of what appears to be a fairly sports‐rugged feel. The hardware paired fairly quickly with a few minor work‐arounds with my phone.  The app was quite friendly, stylish and soothing to the eyes (easy to read)‐ and this is important for anyone who may have a possible brain‐related injuries that call for this type of product. 

SALINE SOLUTION FOR THE ELECTRODE CONTACTS
I provided an alternate solution to the factory's sponge dropper (which is hardly controllable and tends to spill and drip everywhere).  Instead, I employ my Saline Nasal Mist Spray, which dispenses a much more controlled and even amount of moisture to the contact points without any mess.  The App will not let you continue unless all 6 contact points are moistened, and my nasal spray satisfies the App every time!

BRAIN GAMES
Once  paired,  the  app  takes  on  a  life  of  its  own.    It  speaks  to  the headband to set me up, starting with BRAIN GAMES. The head band will read my 'brain waves' to create a BASE LINE of my personal metrics at the start point of all readings. I'm told the original set of tests (through brain games) was reduced from 40 minutes to 10 minutes, which is much less painful  (undergoing 40 minutes of challenging brain games may just be too much for the many impatient people and those with potential head injuries or cognitive disorders.  These games offer 3‐5 sets of memory and focus challenges like staring at an X for 3 Minutes, or remembering a combination of letters and numbers under a limited time.  

ANXIETY
I took on the full Neurovine experience as any other head patient of a physiatrist or neurologist would  undergo an EEG exam. Simulating this has some elevation in stress or anxiety with these brain games, which I was concerned may affect its accuracy and ultimate  results. My instructor  (Yvonne) reassured me that any minor distractions are actually anticipated by the AI whereby the data I collect will have the propensity to auto‐adjust itself to find my median. (God bless AI) 

COMFORT AND WEARABLE
As the device is fairly comfortable and workable with daily life, making the 'art' of collecting cognitive biometrics a breeze.  After a minute or so, you won't even know it's on. It feels just like  any  other  sports  headband  or  baseball  cap  where  the  contact  points  do  not  feel  like anything.  The  APP  showed  default  activities  to  choose  from  (like  driving,  computer  work, listening to music etc) and you can add to them at will. As a personal device, I'll be collecting enough metrics in no time.

HEALTH TECH REPORTER: Test-Driving SKLIP- the Portable Cell-Friendly Endoscope



INTRODUCTION

MY EXPERIENCE WITH SKLIP  By: Roberta Kline, MD

When I was practicing as an ObGyn physician, one of the more challenging dilemmas was determining the urgency of referrals for suspicious skin lesions that were outside my area of expertise – particularly because the typical wait time for an appointment with a dermatologist was at least 3-6 months. Who needed to be seen quickly, and who could watch and wait?

But physicians aren’t the only ones looking at skin. In fact, most skin cancers are initially detected by non-physicians or people themselves. One of my children had a precancerous skin lesion noted by a hair stylist at age 8. A number of years ago, I had a skin lesion on my back that turned out to be cancer. In both cases, had we not detected these early, the outcomes could have been very different.

I was introduced to SKLIP through Mary Nielsen of Spectrum Aesthetics, where she was using the device in her aesthetics training programs. I immediately saw the potential value of the technology to enable early detection of precancerous or cancerous skin lesions, and determine who needs to see a dermatologist more urgently.

It soon turned out I was able to test drive the device myself when I noticed a new skin lesion on my face. My next regularly scheduled skin check was months away. Should I call for an appointment sooner, or could it wait? Now I could get answers quickly using SKLIP.

I opened the box with the dermoscopy lens, mineral oil, and instructions. I then downloaded the app, finding it was easy to set up and navigate. The instructions for preparing the lesion for scanning, charging and attaching the dermoscopy lens, and then taking the image were also clear. There’s also a lot of great educational information about skin.

But actually getting a good image was a little more difficult. I had to do this using a mirror so I could place the lens correctly, and it took me a few tries before I realized the icon for my camera app was hidden by the lens so I needed to turn it on before attaching it to my iphone.

The app then gives immediate feedback as to whether the image is of sufficient quality. It asks for some basic information before submitting the lesion for analysis, including history of skin cancer and a written description of the lesion. 

Despite it being a weekend, I got a report back in less than 24 hours. You can see a sample report below.

This report prompted me to contact my dermatologist right away, rather than “wait and see”. I was able to use this report to be seen quickly for a biopsy. My dermatologist was impressed with the technology for improving early detection and knowing when people needed to be seen more urgently. 

Luckily, my lesion was caught early enough that it had not yet turned to cancer, and I was able to avoid potentially disfiguring surgery to my face. And going forward, it will be so much easier to check out any new moles or skin lesions that crop up in between regular checkups.

Skin cancer guidelines recommend doing a skin check monthly, but often people do not follow these guidelines for many reasons – including not really knowing what to look for, or the stress of finding something and not knowing what to do next. Having the SKLIP makes it easy for people to incorporate this into their health routine. 

The only potential downside is that you have to be able to place the dermoscopy lens directly on the lesion. If you have a lesion on your back, or other location that is not easy to reach or see directly, you would need someone else to help you. 

Beyond personal use, there are so many other professionals who see clients and their skin on a regular basis and could easily incorporate this device into their work: massage therapists, aestheticians, hair stylists, makeup artists, cosmetologists just to name a few. Even other healthcare professionals can use it to better triage referrals to dermatologists. 




"HERX IS NOT A SIDE EFFECT!" By: Josh Schueller, PT

As non-invasive treatments such as electromagnetic and neuro-stimulation devices have shown significant evidence as a safe and effective option for wellness and pain relief within recent decades, one possible bodily reaction may occur that can feel adversely as pain or discomfort- possibly being misunderstood as a side effect of the treatment.  This physiological reaction is called Herxheimer Reaction. INTEGRATING NON-INVASIVE TECHNOLOGIES

Years ago, I learned about biofeedback and electromagnetic field therapy. The market was robust with choices, but something called ONDAMED® won me over- largely due to the functional health strategy by design, indicating a heavy focus on a whole-body care. DEEP SCIENCE NEWS: BREAKING NEW 'MEDTECH' GROUND IN PAIN HEALING

In our ongoing journey to review and feature NON‐INVASIVE MEDICAL & HEALTH INNOVATIONS, IPHA‐NEWS editors recently met with founders of AVACEN® Medical‐husband and wife team CMO/Co‐founder Danielle Forsgren and Chief developer Thomas Muehlbauer. They presented at a Health & Wellness Conference in Miami in the summer of 2022 where they shared a most unique therapeutic design to address chronic and acute pain through the science of microcirculation.


HEALTHTECH REVIEW on AVACEN® MICROCIRCULATION THERAPY

Abbreviated from "Advanced VAscular Circulatory ENhancement", the origin of this groundbreaking innovation was sparked by a deeply personal mission from its founders. In 2008, Tom (Muehlbauer) committed to the research and development of a medical device that could help (and restore) Danielle's older sister who, for 25 years, had suffered the crippling symptoms of severe chronic MIGRAINES. "It was very important [to us] that we had a medical device that was non‐invasive, because my sister took many medications on a daily basis‐ and I actually saw her NOT getting better ‐ but instead, actually grow worse", said Danielle.

It is also noted that the development duo was inspired to target MIGRAINES as the foundation of their main work venture due to our existing HEADACHE and MIGRAINE epidemic (over 20.0% of women and 9.7% of men in America)[1]. At the time, Tom was working with a group of scientists at a major research University on human thermal regulation. He applied this understanding of biophysics to developing the AVACEN concept. 6 years later, the AVACEN device achieved FDA class II clearance to offer thermo therapy for aches, pain, strains, overall muscular relaxation and joint pain associated with arthritis. This gave them the official green light to promote AVACEN as a medical grade device for the countless pain sufferers in the U.S.. In addition, this unique concept has also shown, in a published peer reviewed pilot study [2],  significant Fibromyalgia pain reduction and functioning markers after 4 weeks of directed use.

THE SCIENCE: MECHANISM OF ACTION
With over 4,000 programmed microprocessor instructions and 17 US and International patents, the AVACEN device, uses heat and vacuum, to accomplish the noninvasive and safe infusion of heat into the circulatory system. The result is the dilating of micro vessels thereby increasing microcirculation. It does so by taking advantage of a unique arrangement of blood vessels called arteriovenous anastomosis (AVA’s) in the palm of the hand. "Tom found a unique way to engage the AVA's in the palm of your hand using a dry heat transfer pad and a vacuum", explains Danielle. "The vacuum keeps these AVAs distended, whereas without a vacuum your body would shut the heat transfer down using vasoconstriction. The continually warmed blood is then circulated naturally to all the organs and deep muscle tissues carrying with it an increased level of beneficial oxygen and nutrients. This is also believed to enhance enzyme function for those with low core body temperature, and increase microvascular circulation throughout the entire body. ’ Studies have shown that increased microcirculation can increase functional capillary density (FCD)‐‐ and that is the holy grail for longevity".

PHILOSOPHY:
CMO Danielle Forsgren expressed subscribing to a more EASTERN (and naturopathic/ osteopathic) than WESTERN medicine philosophy. This very framework led the strategy of a sustainable and non‐invasive design plan (ie. "... thermography vs. an x‐ray"). "Having my sister handed pill after pill after drug after patch‐ then a medically implanted box that went septic... we sought to do what we could to get her off of medication and give her a little bit of her life back. That was what Tom and I decided was most important."

INDUSTRY UPGRADES:
Over the years, AVACEN continues to UPGRADE their original design concept with various models to suit ongoing market demands. Their base models called the AVACEN®HOME™ and the HOME+ is a single button push device than accommodates 95% of hand sizes. After five years in development, this year the AVACEN PRO+ and AVACEN XL were launched‐ with color touch screen controls, audio and video applications and a fully integrative user‐interface with various languages including the Asian community. Overview: The Science of Energy Healing

Energetic healing has been practiced by many cultures and in many forms over the centuries. While mainstream medicine has embraced some forms of energetic modalities, by and large it has viewed most as “alternative” and “non-science based”. This is slowly changing as the science is catching up to be able to explain this ancient practice.  The term "energy medicine" has been in general use since the founding of the International Society for the Study of Subtle Energies and Energy Medicine in the 1980s.  Energy medicine often proposes that imbalances in the body's "energy field" result in illness, and that by re-balancing the body's energy-field health can be restored.[5] Some modalities are described to "bring balance to the body's overall wellness", while others bring claims of  addressing pain, detoxing, cell regeneration or improve overall energy. 

ENERGY IS EVERYWHERE
Everything is energy. Quantum physics has demonstrated that everything exists as a vibrational energy. Some forms of energy have a frequency low enough to make the object solid and therefore visible and physically tangible, such as concrete, trees, mountains, and the human body. Others, such as sound, light, heat and gravity are invisible – but they are no less real.  Every living thing requires energy to exist and function, and this comes in different forms. For example, in the human body are “energy generators” called the mitochondria. These tiny powerhouses in our cells convert fat, protein and glucose into ATP – the energy that fuels everything that is needed for us to be alive. THE SITTING CULTURE: HIGH RISK OF DECLINE AND MORTALITY

INTRODUCTION

Amidst our global pandemic, keeping the doctor away means heeding his or her lifestyle advice on prevention, early detection and taking in their research on what it means to truly stay out of trouble.  Meanwhile, upon review of many Americans' health challenges and concerns, we assembled some of the most pertinent questions and concerns that weigh in on their decision making process.   

Months into a new year awakens great opportunities to invest in HEALTHY CHANGE in your lifestyle.  For the many Americans who live a sedentary lifestyle, most of us use time markers (like a new year) as a sensible time to re-dedicate to new challenges.  Losing weight is the number one goal each year for most people - and the undisputed culprit to a non-active lifestyle drives unhealthy choice-making into a situational life trap.  Meanwhile, other members of the community are situated with the inability to perform physical activities due to chronic pain or a debilitating disease (ie. spinal cord injury, traumatic brain injury or stoke sufferers).  

In an interview with Dr. Jonathan S. Kirschner, physiatrist at HSS (Hospital for Special Surgery),  we found his expertise, his thought process and his insight on certain health solutions to deliver remarkable value to us all- especially those affected in some way by the debilitating effects of the pandemic lifestyle.  


2022 LIFE EXTENSION REVIEW

During the COVID pandemic, I see two types of people; people who move less and those who move more.  Some people with more free time typically don't have to spend extra hours to commute.  Now, they've got that time for themselves (instead of commuting) working from their home office.  So that frees up more time for physical activity.  Upon revaluing life and lifestyle in general, some people have the time to commit to things like learning about the importance of health and actually take actionable steps.  Where people did not used to have time to join an exercise program, we now have the option (because of Covid) to watch exercise classes through Zoom and things like that.  

There's enough data pointing the sedentary lifestyle as the main culprit to a wide array of medical issues- not only musculoskeletal issues like back pain, herniated discs, neck pain, neck strain with everyone at the computer because of covid, but also the metabolic effects of sitting.  So if we’re not moving, we're at a high risk for gaining weight, developing diabetes and high blood pressure.  Those are some of the consequences of our modern lifestyle with all this technology and computers, whereas in the old days we had to forage for food.  We had to farm the food, lift bales of hay, walk to the water well and come back home.  We are not doing those things now.  

The best way to communicate all this is to not throw it out to the patient all at once; sometimes it’s better to make baby steps because it can be daunting.  If the patient has a lot of weight to lose, or for anyone who has not been exercising, you want to start engaging them in an exercise program. WHAT? YOU NEVER HEARD OF KREBS CYCLE? (The  Science of Self Regeneration)

As a Physical Therapist and a patient analyst, I find myself constantly referencing course work from my college days to explain to patients how modern therapy actually works.  Today's patient has a hungry mind equipped to handle deeper explanations about how their therapies actually affect them.  Because of this, it behooves me to share content from my own medical texts to explain how the latest "invisible" therapeutic innovations work on them.   Such modern breakthroughs include: Stem Cell & PRP therapies, PEMF & Biofeedback technologies, Transcranial Stimulation, Laser Acupuncture, Neurostim therapeutics etc-- just to name a few. Meanwhile, I am equally impressed at our Google surfing community that can retain much of the same medical information my career was founded on.  Just the other day, a 21 year old patient with spinal cord injury actually started asking me how my neurostim device affects cellular regeneration and THE KREBS CYCLE.  So my presentation begins.


The Krebs’s Cycle is a multi-step chemical reaction that is the main source of energy for cells during Aerobic respiration.  Also called the TCA (T
riCarboxylic Acid) Cycle or Citric Acid Cycle, the Krebs cycle occurs in eukaryotes (cells with a nucleus) in the matrix of the mitochondria and the cytosol of prokaryotes (no distinct nucleus).   In a living organism, the TCA cycle is a series of reactions in which oxidation of Acetic Acid/glucose provides energy for storage in phosphate bonds (ATP).  Fuel for this comes from lipids and carbohydrates, which produce Acetyl-CoA.  These reactions are the major source of energy in all living organisms. 

Critical in the building blocks for synthesis of fatty acids, steroids, cholesterol, amino acids for building proteins and purines and pyrimidines used in the synthesis of DNA. The byproducts of the Krebs’s Cycle are 3 NADH, 1 FADHL, 1 GTP, and 2 CO2. (3)  The cycle runs twice for glucose and then the electron transport chain produces ATP. The electron transport chain is a series of reactions inside the cell whose purpose is the formation of ATP.  By studying the rates, by-products, enzymatic activity and qualities in the processes of metabolism the potential to draw conclusion about and investigate the efficiency of therapies.

Disorders of the Krebs Cycle such as 2-Oxoglutaric aciduria and fumarase deficiency affects mitochondrial function and maintenance.  This can cause developmental delays, severe neurologic problems in infants like mental retardation.  Clinical presentation of mitochondrial depletion syndrome is hypotonia, progressive dystonia, muscle atrophy and hearing impairments.
